In consideration of the services/activities provided by Big South Fork Adventures LLC, their officers, agents, employees and stockholders, all other people or entities associated with those services/activities (hereafter collectively referred to as BSFA), I the Minor, Guest, Parent and/or Guardian agree as follows:​

Although BSFA has taken reasonable steps to provide the minor and/or I with appropriate inspected equipment, procedures, training and skilled-trained guides, in relation to guided and unguided BSFA activities, so, we may enjoy an activity for which we may not be fully skilled for, BSFA, has informed me this activity is not without inherent risks.  These inherent risks are some of the attributes/elements that contribute to the unique character of this activity and can cause the loss of life, permanent physical and mental trauma, loss of personal equipment, lasting injuries and illnesses.

​​In consideration of the stated inherent risks of injury while participating in any BSFA activity and as consideration for the right to participate in such activity and its related events and activities,   I, the Customer, Guest, Participant, Minor, Parent and/or the Guardian, KNOWINGLY ACKNOWLEDGE THE INHERENT RISKS WITHIN THE BSFA ACTIVITIES AND AGREE THAT:  The risks of injury from activities involved in the activity is significant, including the potential for permanent paralysis and death, and while skills, equipment, and personal discipline may reduce these inherent risks, the risks of serious injury exist;  and

1. I ________________________________________________________, (Parent and/or Guardian), KNOWINGLY ACKNOWLEDGE THE INHERENT RISKS both known and unknown, listed and unlisted and I voluntarily consent to the minor’s participation with full awareness of those inherent risks associated with the BSFA activity.

2. I and the minor agree to comply with the stated and customary BSFA terms and conditions for the activity participation.

3. The minor and I have watched and fully understand the Whitewater Safety Video,  (https://www.youtube.com/watch?v=EljMIFKrWq0) and will properly wear the assigned PFD and safety helmet and/or any other prescribed BSFA safety equipment while participating in activity.  If, however, I and/or the minor observe any hazards during our participation, we will remove our presence from the activity and bring such issues to the attention of BSFA trained guide immediately.  In addition:

4. I and/or the minor will not stand up in swift water and will actively participate in our rescue.

5. I and/or the minor understand in unguided/self-guided activities, I, the parent and/or guardian, are responsible for our actions and knowing the activity’s inherent risks and our self-rescue, if necessary.

6. I and/or the minor understand BSFA “On Water” activities are a dynamic and any personal items we bring on the trip could be broken, lost and irretrievable forever.   

7. If BSFA rented equipment is used, I and/or the minor will safely return the equipment in the same state that it was issued, outside of normal activity "wear and tear" in junction with the activity equipment return policy. 

8. I and/or the minor understand that during the activity, we may encounter various wildlife, including poisonous snakes, and insects and we will treat all wildlife with respect and will not approach or handle wildlife. 

9. I and/or the minor acknowledge we are all stewards of the state and/or national parks and forests systems and will not participate in the destruction of such property, including not sleeping or relieving ourselves under rock cropping and littering.   

10. I am responsible for our mental and physical qualifications to participate in the BSFA activities, I certified we are physically and mentally capable of successfully completing this activity. 

11. Big South Fork Adventures LLC has the right to cancel any trips that are deemed unsafe and will offer customary refunds or rescheduling of the trip based on the reason.   No refund will be offered for "No Shows". The consumption of drugs or alcohol is not allowed on any Big South Fork Adventure's expeditions, Activities and/or trips. 

12. I know that whitewater trip's locations are extremely remote, and any emergency services needed will not be immediate, and the parent and/or the participant may be responsible for emergency rescue and medical costs associated with those emergency services 

I HAVE READ AND UNDERSTAND THIS “ACKNOWLEDGEMENT OF INHERENT RISKS” AND VOLUNTARILY ACCEPT THESE INHERENT RISKS ASSOCIATED WITH OUR PARTICIPATION IN THE BSFA ACTIVITY.
